October 18, 2010, Warren, Vt.., Schempp-Hirth Ventus B/16.6

At approximately 1715 Eastern time, the glider was substantially damaged during a forced landing. The private pilot was not injured. Visual conditions prevailed for the flight, which originated with a tow at about 1400. After using ridge and thermal lift to reach 14,000 feet, the pilot used the dive brakes "quite a few times" to maintain a 500-fpm descent. The pilot entered the traffic pattern and was maneuvering on the downwind leg when he experienced a strong lifting force.

June 15, 2010, Parowan, Utah, Schleicher Ash 26E/Schempp-Hirth Ventus 2CT

The two gliders were involved in a midair collision at 1533 Mountain time. Neither pilot was injured. Both pilots were the sole occupants of their respective gliders. The Ash 26E sustained substantial damage, but the Ventus 2CT was not damaged. Both pilots were operating in visual conditions. Both gliders departed Parowan about three hours prior to the collision, and Parowan was the planned final destination for both pilots.

May 4, 2008, Ephrata, Wash., Schempp-Hirth Nimbus-4M

This was the first flight after removing the motor glider from storage and reassembling it. During initial climb, the glider started rolling left despite the pilots control inputs. He aborted the takeoff and landed straight ahead while attempting to maintain control of the powered glider. However, the left wing struck the ground and the glider began to cartwheel. The glider was substantially damaged and its pilot seriously injured.
