July 3, 2010, Fairfield, Penn., Piper PA-25-235/Schleicher ASW-27

At about 1315 Eastern time, the Piper collided with the stationary glider, N747GW while landing. Both aircraft sustained substantial damage. There were no injuries. Visual conditions prevailed. The Pipers pilot reported he "couldnt get the brakes to work" after touchdown and intentionally ground looped the airplane to prevent running into people and other parked aircraft.

June 15, 2010, Parowan, Utah, Schleicher Ash 26E/Schempp-Hirth Ventus 2CT

The two gliders were involved in a midair collision at 1533 Mountain time. Neither pilot was injured. Both pilots were the sole occupants of their respective gliders. The Ash 26E sustained substantial damage, but the Ventus 2CT was not damaged. Both pilots were operating in visual conditions. Both gliders departed Parowan about three hours prior to the collision, and Parowan was the planned final destination for both pilots.

At approximately 1730 Mountain time, the glider was substantially damaged when it impacted terrain, fatally injuring the Commercial pilot. A witness observed the glider enter a left bank to the left, level, then initiate another left turn. The witness stated that the left wing dropped and the glider dove straight to the ground. The glider then disappeared behind a ridge.
